Elevate Physical Therapy was conceptualized and constructed by Manisha Patel MS PT, and Dr. Shivani Patel PT DPT, as a place for clients to receive experienced, 1:1 physical therapy care, in an environment that strives to cultivate and nurture healing. Elevate believes that the more you know about what is going on with your body, the more empowered you feel to control ailments, no matter how acute or chronic.

 

COVID MESSAGE: As Covid-19 numbers continue to rise in our area, the need to feel and be safe while tending to your important physical health ailments, is more important than ever.  At Elevate, our private clinic allows for complete control of who enters and exits the facility, and how many people are in the space at once.  This helps to significantly limit exposure, and can also ease any extraneous concerns of contracting viruses,  and allows you to focus on getting better, in an environment that is clean, safe, and quiet.

 

Having spent most of our careers working in private chain clinics and large hospitals in the Chicagoland area, we have learned that it is important for physical therapists to be able to treat patients the way the profession dictates, not the way a big companies with productivity standards dictate. We are physical therapist owned and operated, and believe that this should be the standard. Our model affords us the opportunity to spend quality one on one time with our patients so that thorough exams and treatments can be conducted. We don’t waste time with treatments that don’t have support in evidence, and we have a multitude of tools in our belts to help you to get back to what you love.

 

Physical Therapy has also successfully evolved to embrace a holistic approach to pain management, and injury prevention. Elevate believes that being educated on what is making you hurt, is the key to being able to manage and control it.

Sports Injury

We are dedicated to sports medicine and focus our rehabilitation on the performance needs of our athletes as well as the prevention of sports-related injuries.

Chronic Pain

Chronic pain is defined as pain that lasts at least 12 weeks. The pain may feel sharp or dull, causing a burning or aching sensation in the affected areas. It may be steady or intermittent, coming and going without any apparent reason. Chronic pain can occur in nearly any part of your body.

Clinical Pilates

Pilates is an exercise regime designed to stimulate the deep spinal muscles, or core muscles to function more efficiently.

Who We Are – Manisha and Shivani share over 30 years of experience working as physical therapists in the Chicagoland area. This extensive experience has allowed them to practically learn and implement treatment based on best practices, and gold standard levels of care for musculoskeletal and orthopedic pain and injury management.

Mechanical Assessment and Functional Treatment Of:

 

Spine

Lumbar, cervical, and thoracic pain, radiculopathy, stenosis, instability, post-surgical management of discectomy and ACDF, minimally invasive spine surgery, spinal fusion, laminectomy, cervicogenic headache, tension migraines.

TMJ Dysfunction

Symptoms include: pain at the jaw or near the ear, difficulty eating hard foods, clicking, locking or popping of the jaw, neck pain and/or headaches, and pain associated with yawning, eating, bruxism or laughing. Treatment includes: hands on soft and deep tissue manipulation, strengthening of the jaw and neck muscles and postural education and exercises.

Shoulder Pain

Rotator cuff syndrome, tendonitis, tendinopathy, impingement syndrome, weakness, nerve entrapment, post-surgical management of rotator cuff repair, arthroscopic shoulder decompression and clavicular excision, total shoulder and reverse total shoulder replacement, Grade I-III AC joint sprain rehab, sports related shoulder injury.

Elbow/Hand Pain

Medial and lateral epicondylitis (Golfers and Tennis elbow), post surgical fracture/ligament repair.

Hip Pain

Pre and post hip replacement surgery, labral tears, arthroscopic repair, hip impingement, hip instability, ITB syndrome, piriformis syndrome, tendinopathy, tendonitis, bursitis, fracture, post surgical management of open reduction internal fixation surgery (ORIF), arthritis.

Knee Pain

Patellofemoral pain syndrome, tendonitis, tendinopathy, post surgical management of ACL reconstruction, meniscectomy, meniscal repair, quadricep and patellar tendon repair, pre and post joint replacement rehabilitation, chronic knee conditions, sports related knee injury, running injury, arthritis.

Foot/Ankle Pain

Ligament sprain, post surgical management of fracture repair, reconstruction, Achilles tendon repair, ligament repair, plantar fasciitis, sports/running related injury.

Chronic Pain

Conditions include fibromyalgia, Ehlers-Danlos Syndrome, Chronic Regional Pain Syndrome, general pain and discomfort, weakness, fatigue, sensitivity to pain, altered somatosensation, headache. Treatment Includes: Individualized treatment plan formatted in collaboration with your practitioner including graded motor imagery, and graded return to exercise and activity.

Do I Need a Prescription for Physical Therapy

In Illinois, a doctors referral or prescription is no longer required to begin therapy services. This allows you direct access to our services without having to see your physician first, so you can get started getting better, faster.

Do You Accept Insurance?

Yes. We accept Blue Cross Blue Cross Blue Shield PPO plans, as well as Medicare.

What if I Don't Have Either BCBS or Medicare, or Don?t want To Use My Insurance Because I Have a High Deductible?

We offer competitive pricing for PT service packages. This allows you to actively manage your health, without having to worry about the restraints insurance companies can present with.

These packages usually cost less than the average insurance deductibles, and allow more freedom with services. Keep in mind, our packages cost less than most insurance deductibles, do not have restrictions on what and how conditions can be treated.

You can purchase:

45 minute session: $150

6 45 minute session: $810

**Packages of 6 or more 45 minute sessions are offered at a discount of10%.

What Can I Expect From My First PT Session?

Your first session, or initial evaluation, is where you and your therapist will have an in depth discussion about what brings you in to see us, and what your goals are for wellness. A thorough history is taken, and appropriate orthopedic movement examination is conducted to determine possible causes for your pain or injury.? From there, we discuss projected length of care, education on your injury, and provide you with a customized home exercise program to begin working towards your goals.

What is Your Cancellation Policy?

We require 24 hours prior cancellation. A $50 late cancellation/no show fee is applied.

What Should I wear to Physical Therapy?

Wear comfortable clothes loose fitting clothing and gym or comfortable shoes that you would work out in.
